On Thursday, May 9th, beginning at 8:30 a.m., Community Healthcare System of Northeast Kansas will be at BVHS to conduct physicals for any 5th-12th grade students for the 2019-2020 school year. These physicals will be provided to our students FREE OF CHARGE! This is a great opportunity to check an item off your summer to-do list early. Last year we had 66 physicals conducted.
Community Healthcare System of Northeast Kansas will be providing a male doctor and female doctor to conduct the physicals along with two nurses. Our school nurse Maggie Haag will also be helping by gathering height and weight measurements for our students. Any student that will be participating in a Physical Education Class throughout middle school or high school,(PE, Weights, Fitness for Life) or participating in an extra curricular activity will need a physical for the 2019-2020 school year.
Community Healthcare System of Northeast Kansas has requested that each student that would like to get his or her physical completed on May 9th needs to have Form-PPE completed with a Parent's signature. You will need to complete the PPE Pre-Participation History Form. Community Healthcare System of Northeast Kansas and BVHS will complete the other requirements and send the physical home with the student once a copy has been made. Please complete the 1st and 4th pages of the physical and make sure they have been signed by the student and parent and have your student bring them on May 9th. The Concussion Form that is required for any student participating in a KSHSAA activity to be signed by the student and parent each year is below.
